On May 28th, the Consumer Voice's Sara Cirba and Amity Overall Laib presented in a webinar hosted by the National Legal Resource Center entitled "Supporting Consumers Who Transition Out of Nursing Homes." The Consumer Voice interviewed numerous individuals who transitioned back to the community and heard from them about what worked, what didn't, and what could have been done better. This webinar covered what consumers said about the transition process and strategies for improvement. The webinar also looked at the roles of care partners and others, such as the long-term care ombudsman and transition coordinators, in supporting the individual.
